In India, it is mandatory under the Registration of Births and Deaths Act, 1969 to register every birth/stillbirth with the concerned State/UT (Union Territory) Government within 21 days of its occurrence. Birth registration is an official evidence of legal recognition of the child as a member of society. It enables the child to prove the date of birth, join the school, enter in to a valid employment contract, prevent child exploitation, to apply passport, to apply ration card entry etc. The birth has to be registered with the Kochi Municipal Corporation within 21 days of its occurrence, by filling up the form prescribed by the Registrar. Birth Certificate is then issued after verification with the actual records of the concerned hospital. For Non-Institutional Birth incidents, a field verification will be conducted by the Health Inspector to verify the details given by the citizen. In case the birth has not been registered within 1 Year of its occurrence, the certificate is issued after RDO approval.

6. Notarized affidavit jointly applied by Parents regarding the Birth and Address of Parents at time of Birth in a Stamp Paper of Rs.100 

7. Copy of following documents of the Parents: 

  • School Certificate of Parents
  • Ration Card
  • Voters Identity Card

8. 2 Witness certificates including copies of following documents of the Witness:

  • Voters Identity Card

  • Place of Birth and Date of Birth of all siblings

Certificate - When Date of Birth is before 1970

2. Stamp Paper of Rs.10 in Applicant's name

3. School Certificate of the Child attested by Gazetted officer

4. Order of Birth form for people living within the Corporation limits

5. Order of Birth Identification certificate for people living outside the Corporation limits

6. For registrations coming from Citizens living outside Kerala, Notarized statement showing:

  • Address of Parents
  • Place of Birth and Date of Birth of all siblings

7. Request to Chief Register with Court fee Stamp of Re.5
